UnitedHealth Group

UnitedHealth Group is a publicly traded health care company (NYSE: UNH) headquartered in Eden Prairie, Minnesota. It runs two businesses: UnitedHealthcare, the health insurer that served 48.5 million people in mid-2026, and Optum, which spans physician practices and care delivery, the Optum Rx pharmacy benefit manager and the Optum Insight data and technology unit. It appears here not as an AI vendor but as a large health insurer and care provider AI adopter, although Optum Insight does sell AI-enabled software, such as automated coding and prior authorization tools, to other insurers and hospitals.

Its defining pattern is operations-scale adoption: instead of one flagship model or cloud partnership, AI is pushed into the huge volume of routine transactions it handles, from claims and prior authorization to call centers and clinical paperwork. In April 2026 management said the company remained on track to invest nearly 1.5 billion dollars in AI-related initiatives in 2026, and told analysts it expected a "conservative" two-to-one return over the next few years. UnitedHealth says it has more than 1,000 active AI use cases, run on an internal platform called United AI Studio.

In March 2026 UnitedHealthcare launched Avery, a generative AI assistant for members' questions about coverage, costs and claims, planned to reach 20.5 million people by year end. In April 2026 the company reported that Optum Real, an Optum Insight product for claim adjudication and coverage checks, can cut manual contact costs by 76 percent, and that an automated prior authorization check at Optum Rx had reduced prescription approval times from more than eight hours to under 30 seconds. By July 2026 AI ambient listening, which drafts clinical notes from a doctor's conversation with a patient, was available to 70 percent of Optum's employed clinicians, with a goal above 90 percent by year end.

Its use of algorithms in coverage is also contested in court. In Estate of Lokken v. UnitedHealth Group, a proposed class action filed in November 2023 in Minnesota federal court, families of Medicare Advantage members allege that the company used nH Predict, a tool from naviHealth, a post-acute care business Optum acquired in 2020, to end nursing home and rehabilitation coverage early and in place of physicians' judgment; the complaint claims the tool has a 90 percent error rate. In February 2025 the judge dismissed several state-law claims as preempted by federal Medicare law but let the breach of contract and good-faith claims proceed, and in March 2026 a magistrate judge ordered broad discovery into how nH Predict was designed and used, while declining to require its source code and data. These are allegations, not findings. UnitedHealth has said nH Predict is a guide for planning a patient's care, not a tool that makes coverage decisions, and states that AI is not used to make clinical denials, each of which is reviewed by a qualified medical professional.

The honest framing: UnitedHealth's value rests on insurance premiums, care delivery and pharmacy services, with AI as a cost and service lever on top. The planned spending and the two-to-one return are intentions, most published results are company-reported, and the Lokken case shows the legal and reputational risk of automating decisions about people's care. It differs from CVS Health, its closest peer in the catalog, in also selling its AI tools to other insurers and hospitals through Optum Insight, and from pharma adopters such as Merck & Co., which bet on one Google Cloud platform.
